Cooperative Education

Cooperative education is a method of instruction that enables students to combine academic classroom instruction (school-based learning component) with occupational instruction through learning on the job (work-based learning component) in a career area of choice. Emphasis is placed on the students’ education and employability skills.

The clearances listed below must be completed and returned to Central Westmoreland Career and Technology Center, Attention: Debbie Grindle, 240 Arona Road, New Stanton, PA 15672 prior to any student beginning Co-Op.

1 Pennsylvania State Police Request for Criminal Record Check Form SP 4-164: Free — Volunteer if completed online or by mail. Application can be completed online at https://epatch.pa.gov/home. Click on “New Record Check/Volunteers Only”. Click on the Reason, and then click ‘Volunteer Free’ from the pull down menu. To print the clearance, click on the Control Number link, then Certification Form. This will produce the clearance for printing.

2. Pennsylvania Child Abuse History Clearance  Form CY 113: Free — Volunteer. Application can be completed online at https://www.compass.state.pa.us/CWIS. Creating an account and submitting your clearance application online will give you immediate access to your results or the status of your results if your results cannot be processed immediately. Paper submissions of the application will still be accepted for anyone who may not have access to the internet.

3. Arrest/Conviction Report and Certification Form: Attached, you will find the Arrest/Conviction Report and Certification form. No cost.

4. Act 114 FBI Federal Criminal History Record: Attached, you will find the Disclosure Statement Application for Volunteers form. No cost.

*Only one person needs to have the clearances and they are good for five years.</i

IMPORTANT:

Please be advised that effective at close of business November 18, 2018, Cogent will no longer be providing fingerprint services for ‘new applicants’. All ‘new applicants’ will be required to begin a new pre-registration with the new Commonwealth supplier, MorphoTrust, on November 28, 2018. Those unable to meet this deadline will be required to restart the process with the new Commonwealth supplier, MorphoTrust. (Registration information will not be transitioned to the new supplier; any unused registration files will be purged from the Cogent System and any payments will be issued refunds by Cogent.)

Applicants that have been notified their prints were rejected for quality purposes will have an additional time-period to complete the reprint process with Cogent. Any ‘reprint applicants’ who have not completed the reprint process with Cogent within this additional time-period, will be eligible to continue the reprint process with the new supplier. Please Note, ‘reprint applicants’ will have a limited time-period of 90 days to obtain their reprint services without charge. Additional information regarding that process or the new solution and start date will be forthcoming from the Department of General Services.
